Zia...is for doctor evaluations and getting medical card approval. They take walk in patients. Bring your ID and medical records for the past year. Or call for an appointment to guarantee quick processing.
Employees and Dr. are very nice, explain any questions you have and are very helpful, understanding and empathetic.
Upon completion, they provide a complete packet to mail to the state and give you printed directions to the nearest post office.
